Position: Home Health Services Nurse

As a Home Health Services Nurse at Hospice Compassus, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. You will be responsible for providing skilled nursing care to patients in their homes, ensuring that their physical, emotional, and spiritual needs are met. Your role will involve collaborating with a multidisciplinary team to develop and implement comprehensive care plans that promote comfort and dignity for our patients.

Responsibilities:

• Assessing and evaluating patient needs
• Developing and implementing individualized care plans
• Administering medications and treatments as prescribed
• Providing emotional support to patients and their families
• Collaborating with physicians and other healthcare providers
• Documenting patient care and outcomes

Qualifications:

• Registered Nurse license in the state of [insert state]
• Minimum of 2 years of nursing experience
• Home health or hospice experience preferred
