Job Title: Home Health Nurse

Job Description

We are currently seeking a Home Health Nurse to join our team. The Home Health Nurse will be responsible for providing skilled nursing care to clients in their homes. This includes assessing clients’ health needs, developing care plans, administering medications, and performing various medical procedures. The Home Health Nurse will work closely with clients, their families, and other healthcare professionals to ensure the highest level of care.

Responsibilities

• Conduct initial assessments of clients’ health and develop personalized care plans
• Administer medications and treatment as prescribed by the physician
• Monitor clients’ progress and adjust care plans as needed
• Educate clients and their families on health management and disease prevention
• Collaborate with other healthcare professionals to provide comprehensive care
• Maintain accurate and up-to-date client records

Qualifications

• Registered Nurse (RN) license in the state of [insert state]
• Minimum of 2 years of nursing experience
