Job Title: Home Health Services Nurse

Job Description

We are currently seeking a compassionate and skilled Home Health Services Nurse to join our team. The ideal candidate will have experience in providing nursing care to patients in their homes and will demonstrate a commitment to delivering excellent patient-centered care. As a Home Health Services Nurse at Mena Health Inc, you will be responsible for assessing patient needs, developing care plans, and providing skilled nursing care to patients in their homes.

Key Responsibilities:

• Conduct comprehensive assessments of patients’ health status
• Develop individualized care plans in collaboration with other members of the healthcare team
• Provide skilled nursing care, including administering medications, dressing changes, and wound care
• Educate patients and their families on self-care techniques and disease management
• Coordinate care with other healthcare professionals, including physicians, therapists, and social workers
• Monitor patients’ progress and adjust care plans as needed
• Document all patient care activities accurately and timely

Qualifications:

• Registered Nurse (RN) license in the state of Massachusetts
• Minimum of 2 years of nursing experience, preferably in home health or a related setting
• Strong clinical assessment and critical-thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and collaboratively in a team environment
• Valid driver’s license and reliable transportation
