We Are Hiring: Home Health Services Specialist

Job Responsibilities:

• Provide direct care services to clients in their homes, including assistance with activities of daily living, medication management, and medical monitoring
• Develop and implement personalized care plans based on the individual needs of clients
• Collaborate with healthcare professionals, caregivers, and family members to ensure coordinated and comprehensive care
• Monitor and assess the health and well-being of clients on an ongoing basis
• Document client progress and communicate updates to the care team

Qualifications:

• Valid certification or licensure in a related healthcare field (e.g. Certified Nursing Assistant, Home Health Aide, Licensed Practical Nurse)
• Experience working in a home health setting preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and patient-centered approach to care
