Position: Home Health Services Nurse

Location: Various locations across the United States

Job Description

As a Home Health Services Nurse at Home Health Care Network, you will be responsible for providing skilled nursing care to patients in their homes. You will work closely with patients, their families, and other healthcare professionals to develop and implement individualized care plans. Your primary goal will be to help patients regain and maintain their independence while promoting their overall health and well-being.

Responsibilities

• Conduct initial patient assessments and develop comprehensive care plans
• Administer medications and treatments as prescribed by physicians
• Monitor patients’ progress and communicate with healthcare team members
• Educate patients and their families on managing their health conditions
• Provide emotional support and compassion to patients and their families
• Maintain accurate and up-to-date medical records

Qualifications

• Valid Registered Nurse (RN) license in the state of practice
• Minimum of 2 years of nursing experience, preferably in home health or a similar setting
• Excellent communication and interpersonal skills
• Strong clinical judgment and critical thinking abilities
• Ability to work independently and as part of a multidisciplinary team
• Reliable transportation and a valid driver’s license
