Position: Healthcare Clinician

Job Description:

As a Healthcare Clinician at Harmony Healthcare Services, you will be responsible for providing comprehensive care and support to patients in our health and welfare clinics. Your primary duties will include:

• Evaluating patients’ medical histories and assessing their current health status
• Developing individualized care plans and treatment goals
• Administering medications and monitoring patients for adverse reactions
• Collaborating with a multidisciplinary team to ensure holistic care for patients
• Educating patients and their families on health management and wellness practices
• Maintaining accurate and up-to-date patient records
• Participating in quality improvement initiatives to enhance patient care

Qualifications:

To be considered for the Healthcare Clinician position, candidates must meet the following requirements:

• Registered Nurse (RN) or Nurse Practitioner (NP) license in good standing
• Bachelor’s degree in Nursing or related field (Master’s degree preferred for NP)
• Minimum of two years of clinical experience in a healthcare setting
• Strong interpersonal and communication skills
• Ability to work effectively in a fast-paced environment

Position: Hospice Care Coordinator

Job Description:

As a Hospice Care Coordinator at Harmony Healthcare Services, you will play a vital role in providing compassionate end-of-life care to patients and their families. Your responsibilities will include:

• Assessing patients’ eligibility for hospice services and coordinating admissions
• Developing comprehensive care plans that address physical, emotional, and spiritual needs
• Collaborating with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated care
• Providing emotional support and counseling to patients and families facing end-of-life issues
• Facilitating communication and decision-making among care team members and family members
• Ensuring compliance with hospice regulations and documentation requirements
• Participating in interdisciplinary team meetings and ongoing education programs

Qualifications:

Qualified candidates for the Hospice Care Coordinator position must meet the following criteria:

• Licensed Social Worker (LSW) or Registered Nurse (RN) with hospice experience
• Bachelor’s degree in Social Work, Nursing, or related field (Master’s degree preferred)
• Minimum of two years of experience in hospice or palliative care
• Strong communication and counseling skills
• Ability to navigate complex end-of-life issues with sensitivity and empathy
• Proficiency in electronic health records and documentation
