Home Health Services:

Registered Nurse (RN):

• Provide skilled nursing care to patients in their homes
• Assess patients’ conditions and develop individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor patients’ progress and communicate with healthcare team

Home Health Aide (HHA):

• Assist patients with daily tasks such as bathing, dressing, and grooming
• Provide companionship and emotional support to patients
• Assist with light housekeeping and meal preparation
• Follow care plans developed by RNs and therapists

Nursing Homes-Skilled Nursing Facility:

Licensed Practical Nurse (LPN):

• Provide nursing care to residents in a skilled nursing facility
• Administer medications and treatments as ordered by physicians
• Assist with assessments and care planning
• Supervise nursing assistants and provide training as needed

Certified Nursing Assistant (CNA):

• Assist residents with activities of daily living
• Take and record vital signs
• Provide emotional support to residents and their families
• Work under the supervision of LPNs and RNs

Nursing & Convalescent Homes:

Registered Nurse (RN):

• Provide skilled nursing care to residents in a nursing home setting
• Develop and implement care plans for residents
• Collaborate with interdisciplinary team to ensure quality care
• Provide education and support to residents and their families

Medical Director:

• Provide oversight of medical care in the facility
• Collaborate with nursing staff and other healthcare professionals
• Ensure compliance with state and federal regulations
• Participate in quality improvement initiatives
