Position: Home Health Nurse

As a Home Health Nurse with Guardian Home Health Care, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes. You will work closely with patients, families, and interdisciplinary teams to create personalized care plans and ensure the best possible outcomes for each patient.

Key Responsibilities:

• Conduct comprehensive assessments of patients’ health status
• Develop and implement individualized care plans
• Provide skilled nursing care, including wound care, medication management, and infusion therapy
• Educate patients and their families on self-care techniques
• Collaborate with physicians and other healthcare professionals to coordinate patient care
• Monitor patients’ progress and adjust care plans as needed
• Document patient care activities in accordance with company policies and procedures

Qualifications:

• Valid RN license in the state of practice
• Minimum of one year of nursing experience in a clinical setting
• Home health experience preferred
• Strong clinical skills and the ability to work independently
• Excellent communication and interpersonal skills
• Valid driver’s license and reliable transportation
