As a Health Plans-Information & Referral Service Specialist, your responsibilities will include:

• Assessing patients’ needs and referring them to appropriate healthcare providers
• Educating patients on available health plans and services
• Ensuring patients have access to necessary resources for their healthcare needs
• Collaborating with healthcare professionals to coordinate patient care

Position: Home Health Services Coordinator

Location: Multiple locations

We are also seeking a dedicated Home Health Services Coordinator to join our team. This role involves overseeing the coordination of home healthcare services for patients, ensuring that they receive the support they need in the comfort of their own homes.

As a Home Health Services Coordinator, your responsibilities will include:

• Coordinating home healthcare services for patients, including nursing care, therapy, and medical equipment
• Ensuring that patients’ needs are met and that their care plans are followed diligently
• Communicating with healthcare providers, patients, and their families to provide updates and support
• Monitoring and evaluating the effectiveness of home healthcare services

Position: Medical Clinics Manager
