Job Title: Nurse Practitioner

Job Description:

As a Nurse Practitioner at Catholic Health Initiatives, you will play a vital role in providing comprehensive primary care services to patients in our clinics. You will work alongside a team of healthcare professionals to diagnose and treat acute and chronic illnesses, promote wellness, and educate patients on preventive care. Your compassionate care and attention to detail will make a difference in the lives of those you serve.

Key Responsibilities:

• Conduct thorough patient assessments and develop individualized treatment plans
• Prescribe medication and monitor patient response
• Collaborate with physicians and other healthcare professionals to provide holistic care
• Educate patients on managing their health conditions and making healthy lifestyle choices
• Maintain accurate and up-to-date patient records
• Adhere to CHI’s policies and procedures to ensure quality care

Qualifications:

• Master’s degree in Nursing
• Current Nurse Practitioner license in the state of [insert state]
• Certification as a Family Nurse Practitioner (FNP) or Adult Gerontology Nurse Practitioner (AGNP)
• Minimum of 2 years of clinical experience in primary care
• Strong communication and interpersonal skills
