Job Title: Home Health Nurse

Job Description:

As a Home Health Nurse at Cne Home Health Services, you will be responsible for providing skilled nursing care to our patients in their homes. This includes assessing patient needs, developing and implementing care plans, administering medications, and collaborating with other healthcare professionals to ensure the best possible outcomes for our patients. You will have the opportunity to work one-on-one with patients and their families to provide compassionate and personalized care.

Responsibilities:

• Conduct comprehensive assessments of patients’ health status
• Develop individualized care plans in collaboration with the patient, their family, and other healthcare professionals
• Provide skilled nursing care, including wound care, medication management, and symptom management
• Educate patients and their families on healthcare practices and disease management
• Monitor patients’ progress and adjust care plans as needed
• Document all patient care activities accurately and timely

Qualifications:

• Valid RN license in the state of practice
• Minimum of 2 years of nursing experience, preferably in home healthcare or a similar setting
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Valid driver’s license and reliable transportation
