Job Title: Home Health Services Provider

Job Responsibilities:

• Provide personal care services to clients in their homes, including assistance with activities of daily living such as bathing, dressing, and grooming.
• Assist clients with mobility, transfers, and ambulation as needed.
• Administer medications and provide medication reminders as directed by the client’s healthcare team.
• Assist with meal preparation, feeding, and light housekeeping tasks.
• Monitor and report changes in the client’s condition to the supervising nurse or healthcare provider.
• Provide emotional support and companionship to clients and their families.

Qualifications:

• Must have a valid certification as a Home Health Aide or Certified Nursing Assistant.
• Prior experience working in a home health care setting is preferred.
• Ability to communicate effectively with clients, families, and members of the healthcare team.
• Demonstrated compassion, empathy, and professionalism in all interactions.
• Reliable transportation and a valid driver’s license.
• Ability to pass a background check and drug screening.
