We are hiring for the following position:

Care Coordinator

Job Description:

• Manage patient care by coordinating with physicians, nurses, therapists, and other healthcare professionals
• Assess patient needs and develop individualized care plans
• Monitor patient progress and update care plans as needed
• Provide education and support to patients and their families
• Collaborate with insurance providers to ensure coverage for necessary treatments
• Document all patient interactions and maintain accurate records
• Participate in team meetings and contribute to the overall improvement of patient care

Qualifications:

• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Knowledge of healthcare laws and regulations
• Experience in care coordination or a related field
• Degree in Nursing, Social Work, or Healthcare Administration
• Current CPR certification
