We are Hiring! Position: Home Health Nurse

Job Description:

• Provide skilled nursing care to patients in their homes
• Assess and monitor patient’s health status
• Develop and implement individualized care plans
• Administer medications and treatment as prescribed by a physician
• Educate patients and their families about their conditions and self-care

Qualifications:

• Current RN or LPN license in the state of [insert state]
• Minimum of 2 years of nursing experience
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver’s license and reliable transportation
