Job Title: Home Health Nurse

Job Description:

As a Home Health Nurse at CenterWell Home Health, you will have the opportunity to make a meaningful difference in the lives of our patients. You will work closely with patients and their families to develop individualized care plans and provide direct nursing care in the home setting. Your responsibilities will include but are not limited to:

• Conducting assessments of patients’ health status
• Developing and implementing care plans in collaboration with the patient, their family, and other healthcare professionals
• Administering medications and treatments as prescribed
• Providing education and support to patients and their families
• Monitoring and documenting patient progress

Qualifications:

To be considered for the position of Home Health Nurse, you must meet the following qualifications:

• Registered Nurse (RN) with a valid license in the state of practice
• Minimum of one year of nursing experience, preferably in home health or a similar setting
• Strong assessment and clinical skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver’s license and reliable transportation
