Job Responsibilities

As a member of the Crescent Home Health team, you will have the opportunity to make a real difference in the lives of our clients. Your responsibilities may include, but are not limited to:

• Providing skilled nursing care to patients in their homes
• Assessing patient needs and developing individualized care plans
• Administering medications and treatments as prescribed
• Monitoring and documenting patient progress
• Collaborating with other healthcare professionals to ensure continuity of care

Qualifications

To be considered for a position at Crescent Home Health, you must meet the following qualifications:

• Valid state license or certification (RN, LPN, CNA, PT, OT)
• Excellent clinical skills and bedside manner
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Reliable transportation and valid driver’s license
