We are Hiring: Eldercare-Home Health Services

Position: Eldercare Home Health Nurse

At Gentiva Health Services, we are looking for experienced and compassionate Home Health Nurses with a passion for caring for the elderly. As an Eldercare Home Health Nurse, you will be responsible for providing comprehensive medical care and support to elderly patients in their homes. Your primary goal will be to help seniors maintain their independence and improve their quality of life through personalized care and treatment.

Responsibilities:

• Conduct initial assessments and develop individualized care plans for elderly patients
• Monitor and evaluate patients’ conditions, providing skilled nursing care as needed
• Administer medications, treatments, and other medical interventions as prescribed
• Collaborate with other healthcare professionals to ensure coordinated and effective care
• Provide education and support to patients and their families on managing chronic conditions and promoting healthy living

Qualifications:

• Current RN license in the state of practice
• Minimum of 2 years of experience in home health care, preferably with elderly patients
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and as part of a team
