Position Available: Hospice Nurse

Grace Hospice is currently seeking a Hospice Nurse to join our team. As a Hospice Nurse, you will play a crucial role in providing care and comfort to patients with life-limiting illnesses. You will work closely with patients and their families to ensure that their physical, emotional, and spiritual needs are met.

Responsibilities:

• Provide skilled nursing care to hospice patients in their homes or other care settings
• Assess patients’ physical and emotional needs and develop individualized care plans
• Administer medications and treatments as prescribed
• Collaborate with other members of the healthcare team to coordinate patient care
• Provide education and support to patients and their families
• Ensure that all patient records are accurately maintained

Qualifications:

• Active RN license in the state of Illinois
• Minimum of 1 year of nursing experience, preferably in hospice or home health
• Excellent communication and interpersonal skills
• Compassionate and caring attitude
• Ability to work independently and as part of a team
• Strong organizational and time management skills
