Job Title: Home Health Nurse

Job Description:

We are currently seeking experienced and dedicated Home Health Nurses to join our team. As a Home Health Nurse at Gulston Healthcare Inc, you will be responsible for providing comprehensive nursing care to patients in their homes. Your primary role will be to assess the patient’s condition, develop individualized care plans, and administer medications as prescribed. You will also collaborate with other healthcare professionals to ensure the best possible outcomes for our patients.

Key Responsibilities:

• Conduct initial assessments of patients’ health status
• Develop and implement care plans tailored to each patient’s needs
• Administer medications and treatments as prescribed
• Monitor and evaluate patient progress regularly
• Provide education and support to patients and their families
• Collaborate with physicians and other healthcare professionals

Qualifications:

• Active RN license in the state of practice
• Minimum of 2 years of nursing experience
• Experience in home health or a related field preferred
• Strong clinical skills and attention to detail
• Excellent communication and interpersonal abilities
• Ability to work independently and as part of a team
