Position: Home Health Nurse

Job Description

We are currently seeking a dedicated and compassionate Home Health Nurse to join our team. The Home Health Nurse will be responsible for providing skilled nursing care to patients in their homes, including assessing health needs, developing care plans, administering medication, and monitoring patient progress. The ideal candidate will have a strong clinical background, excellent communication skills, and a genuine desire to help others.

Responsibilities

• Conduct comprehensive assessments of patients’ health status
• Develop individualized care plans based on assessment findings
• Administer medications and treatments as prescribed by physicians
• Monitor and evaluate patient progress and adjust care plans as needed
• Educate patients and their families on managing medical conditions
• Coordinate care with other healthcare providers and community resources

Qualifications

• Current RN license in the state of practice
• Minimum of 1 year of clinical experience, preferably in home health or a similar setting
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Valid driver’s license and reliable transportation
