Position: Home Health Services Provider

Overview:

• Provide in-home health services to patients as directed by healthcare professionals
• Assist with personal care, medication management, and other activities of daily living
• Communicate effectively with patients, families, and healthcare team members
• Maintain accurate and up-to-date documentation of patient care
• Adhere to all company policies and procedures

Qualifications:

• Valid certification or license in a healthcare-related field (e.g. nursing assistant, medical assistant, etc.)
• Experience working in a home health setting preferred
• Excellent interpersonal and communication skills
• Ability to work independently and as part of a team
• Strong attention to detail and organizational skills
