Position Available: Home Health Nurse

Job Description:

As a Home Health Nurse with Hospice Plus-Bellaire, you will be responsible for providing skilled nursing care to patients in their homes. You will work closely with patients, families, and a multidisciplinary team to develop and implement individualized care plans. Your primary goal will be to deliver compassionate and high-quality care to ensure the comfort and well-being of our patients.

Responsibilities:

• Conduct comprehensive assessments of patients’ health status
• Develop and implement individualized care plans
• Administer medications and treatments as prescribed
• Monitor patients’ progress and adjust care plans as needed
• Educate patients and families on managing their health conditions
• Collaborate with other healthcare professionals to coordinate care
• Provide emotional support and guidance to patients and families

Qualifications:

• Registered Nurse (RN) license in the state of Texas
• Minimum of 2 years of nursing experience, preferably in home health or hospice
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and empathetic demeanor
• Valid driver’s license and reliable transportation
