Position: Home Health Services Provider

• Location: Houston, TX
• Full-time position

Job Responsibilities:

• Provide care to patients in their homes
• Assess patients’ needs and develop care plans
• Administer medications and treatments as prescribed by healthcare providers
• Monitor patients’ progress and communicate with healthcare team
• Ensure patients’ safety and comfort
• Document care provided and communicate with patients’ families

Qualifications:

• Valid Texas nursing license
• Minimum of 1 year of experience in home health services
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
