We are Hiring!

We are currently seeking dedicated and compassionate individuals to join our team as Patient Advocates. As a Patient Advocate, you will play a crucial role in supporting individuals with developmental disabilities and special needs by advocating for their rights, coordinating services, and promoting independence and inclusion in the community.

Job Responsibilities

  • Provide support and advocacy for individuals with developmental disabilities and special needs
  • Coordinate services and resources to ensure individualized care plans are met
  • Collaborate with healthcare providers, social workers, and families to ensure holistic care
  • Empower individuals to make informed decisions about their care and support options
  • Promote independence and inclusion in the community through education and outreach
  • Document and maintain accurate records of advocacy efforts and outcomes

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Special Education, or related field
  • Experience working with individuals with developmental disabilities and special needs
  • Strong communication, advocacy, and organizational skills
  • Ability to work independently and as part of a multidisciplinary team
  • Knowledge of laws and regulations related to disability rights and advocacy
  • Passion for promoting inclusivity, empowerment, and independence for individuals with disabilities
