Position: Home Health Nurse

We are currently seeking a passionate and dedicated Registered Nurse to join our team as a Home Health Nurse. This position involves providing skilled nursing care to patients in their homes, collaborating with interdisciplinary teams to develop individualized care plans, and educating patients and their families on proper care techniques.

Responsibilities:

• Conduct home visits to assess patient needs and provide skilled nursing care
• Develop and implement care plans in collaboration with the patient, family, and healthcare team
• Administer medications, perform wound care, and monitor vital signs
• Educate patients and families on disease management and self-care techniques
• Document all patient encounters and progress notes accurately and timely
• Collaborate with physicians and other healthcare providers to ensure coordinated care

Qualifications:

• Valid Registered Nurse license in the state of [State]
• Minimum of 2 years of nursing experience, preferably in home health or a related field
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
