Home Health Services- Registered Nurse

Specialty Therapeutic Care, LP is a leading provider of home health services, committed to delivering exceptional care to individuals in need of therapeutic services within the comfort of their own homes. We are currently seeking a dedicated and compassionate Registered Nurse to join our team and help us in our mission of providing high-quality care to our patients.

Position: Registered Nurse (Home Health Services)

Job Responsibilities:

– Conduct initial patient assessments and develop individualized care plans
– Provide skilled nursing care to patients in their homes
– Administer medications and treatments as prescribed by the physician
– Monitor patient progress and make necessary adjustments to the care plan
– Educate patients and their families on disease management and preventive care
– Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care for each patient
– Maintain accurate and up-to-date patient records
– Participate in quality improvement initiatives to enhance patient outcomes

Qualifications:

– Valid Registered Nurse license in the state of practice
– Minimum of 1 year of clinical experience, preferably in a home health setting
– Strong clinical assessment and critical thinking skills
– Excellent communication and interpersonal skills
– Ability to work independently and as part of a team
– Proficiency in using electronic medical records systems
– BLS certification required, ACLS certification preferred
– Valid driver’s license and reliable transportation
