Job Description

We are currently seeking a dedicated and experienced Home Health Nurse to join our team. The Home Health Nurse will be responsible for providing skilled nursing care to patients in their homes. This includes assessing patient needs, developing care plans, administering medications, and educating patients and their families about their conditions and treatments.

Key Responsibilities

• Conduct initial assessments of patients’ health status
• Develop individualized care plans based on assessment findings
• Monitor patients’ progress and update care plans as needed
• Administer medications and treatments as prescribed by physicians
• Provide education to patients and families about their conditions
• Collaborate with other healthcare professionals to ensure comprehensive care

Qualifications

• Registered Nurse (RN) license in the state of Illinois
• Minimum of 2 years of experience in home health or a related field
• BLS certification
• Excellent communication and interpersonal skills
• Strong critical thinking and problem-solving abilities
• Ability to work independently and as part of a team
